About the Job

Objective / Overview :

In pursuit of its mission, Transforming care and education to advance the health of our patients and the communities we serve, CUHCC provides medical, dental, and behavioral health care to nearly 12,000 racially and culturally diverse patients annually. The vast majority of patients live below federal poverty lines and more than half require language interpretation services.

The clinic integrates primary care, behavioral health, and dental services. Our behavioral health department encompasses outpatient psychiatry and therapy, Substance Use Disorder services, and IBH-Care Coordination embedded in primary care. Community programs, including ARMHS and Targeted Case Management, further support whole-person care. The Integrated Care & Community Health Program Manager provides dual clinical and programmatic leadership for CUHCC's Integrated Behavioral Health-Care Coordination (IBH-CC) team and the Community Health Team, composed of community health workers (CHWs).
