Flagstar Rehab is Seeking Occupational Therapists (OT) for Skilled Nursing Facilities!
Flagstar Rehab puts Occupational Therapists in the driver’s seat when it comes to managing their careers and work-life balance. Therapists have the exibility to choose assignments that t their schedules, with the added benet of a “choose the way you earn” model and support in setting up a corporation if they prefer independent work.

This exibility allows OTs to be present for both their families and the patients they serve, while lling critical gaps in geriatric care.

Hourly Pay Rate:
Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)
Occupational Therapist Requirements and Responsibilities:
Current NYS OT License and Registration
Valid BLS Certification
EMR Competence
Meets all State and Federal requirements for US employment in a Nursing Facility
Malpractice/Liability Insurance (if applicable)
Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
Monitor and document patient progress with the use of innovative EMR software.
