Upper Valley Services (UVS) is one of Vermonts designated non-profit agencies serving people with developmental disabilities and adults who are eligible for Adult Family Care.
We are seeking an experienced Direct Support Professional who is considerate, open-minded, creative, provides positive encouragement, willingness to introduce new interests and behavioral support who will be able to support the teams at UVS.
- A $500 hiring bonus will be offered to the selected candidate after successfully completing six months and another $500 bonus after successfully completing one year in the position.
Community & In-home Specific Support
Perform all duties as required, including but not limited to personal hygiene, meal preparation and / or medication administration.Ensure the safety and well-being of all individuals served as identified in the Emergency Fact Sheet (EFS), Positive Behavioral Support Plan and Individual Support Agreement (ISA).Monitor medical and dietary needs as appropriate.Address individual needs in a personalized manner.Provide a stimulating and positive experience in the community based on the interests and preferences of the individual.Provide the necessary transportation to serve individual(s).Foster the growth of relationships among community members through community- based activities and other community opportunities.Maintain accurate data sheets and electronic health recordsbased on the ISA goals weekly.Have access to up to date ISA, EFS, and any other pertinent documentation that is relevant to the individual(s) you are serving.Employment Specific Support
Support individuals at their place of employment.Provide the required level of support to ensure a sustained, successful employment experience for each individual(s).Be an active member of an individuals employment team, including, but not limited to, attending meetings, providing transportation, and acquiring job specific trainings.Foster a natural support system within their working environment.Establish and maintain a professional relationship with individual(s) and employers.Communicate any employment or job site issues immediately to the DSP Manager and Service Coordinator.Ensure that the employment goals as outlined in the ISA are being met.Maintain a flexible schedule to meet the employment requirements of the individual(s).Ability to recognize and implement necessary adaptations to the work environment and job tasks to promote further independence or development of natural work- place supports.Be aware of safety procedures on the job site and know how to support the individual(s) in case of a job site emergency.Maintain accurate data sheets and electronic health records based on the ISA goals weekly.Have access to up to date ISA, EFS and any other pertinent documentation that is relevant to the individual(s) you are serving.Team DutiesParticipate in team meetings, including ISA meetings, as requested.Actively participate in the development of Community and Employment Support, through building community relationships and linking individual(s) to opportunities and supports naturally occurring in their community of choice.Attend required meetings and trainings and provide input on a regular basis.Participate in all scheduled cross-trainings.Cover open shifts that are beyond set schedules and be a supportive team member.Attend DSP Team Meetings as scheduled.Attend bi-weekly supervisory meetings with the DSP Support Manager.The ideal candidate must be able to :Physically able to lift and carry 50lbs.High capacity for self-regulationStrong communication and teamwork skillsMust be 18y / o or olderCandidates will undergo a background check, must have a valid driver's license, reliable transportation and auto insurance.Benefits :Delta DentalCBA BlueHealth savings accountLife insurancePaid time offRetirement planVision insuranceGrowth and DevelopmentDisclaimer :
