Job Requirements :

Experience : Minimum 2 years of case management experience required

License : Active Oregon LCSW license.

Certifications : BLS preferred

Charting System : Epic (required)

Preferred Experience :

• Teaching or community hospital setting
• LTAC
• Trauma Level I or II environments

Travel Experience : Not required
