Providing care coordination and administrative support, the full-time Wellness Coordinator will engage members with behavioral health needs, screen for gaps in care, and develop individual care plans while working remotely in the Kansas City, MO area. Key responsibilities Manage inbound and outbound member communications to identify and address care gaps Document interactions and coordinate information with clinical team partners Provide behavioral health benefit education and connect members with appropriate resources Required qualifications High School Diploma or GED 2+ years of managed care or call center experience 1+ years of experience in a medical or behavioral health setting 1+ years of computer proficiency, including Adobe and Microsoft Office Access to a designated quiet workspace with secure internet for handling Protected Health Information (PHI)
