Job description
Supporting clinical staff in care coordination, the full-time Health Navigator will assist members in accessing medical, social, or behavioral services while working remotely. Key responsibilities Provide support to participants in case and disease management programs to achieve treatment goals Assist members in navigating and connecting to clinical and community resources, including health screening programs Document care coordination activities and maintain communication with participants, caregivers, and providers Required qualifications High school diploma or equivalent education, experience, or training required Minimum of two years of experience in an acute care facility, preferably in case management or emergency department Preferred certifications include Medical Assistant, CNA, or Home Health Aide Knowledge of managed care, including contracted and non-contracted providers Spoken and written fluency in English
