Job Description
Purpose of Position:
To provide individual and group substance abuse counseling and support to patients on the Assertive Community Treatment (ACT) Team. The Senior Addiction Counselor will play an active role in assessing, planning and supporting patients in both their mental health and addiction treatment needs.
Primary Responsibilities:
- Provides comprehensive psychosocial and substance abuse assessments for the patients of the ACT Team.
- Completes home and community-based visits to engage patients in their natural environments.
- Devises patient driven treatment and wellness plans for both new and existing ACT patients.
- Meets individually with patients and collaterals to assist in identifying their own individual warning signs of relapse and identifying and developing individual coping strategies.
- Provides psychoeducation and support to family and collaterals to support recovery goals.
- Documents all encounters with patients and collaterals accordingly in electronic medical record.
- Provides supportive peer counseling with respect to mental health, substance abuse, rehabilitation strategies and self-help opportunities.
- Participates in interdisciplinary treatment and human services team in determining and implementing a comprehensive plan of care.
- Clarifies issues for the consumer and assists with the process of referral for treatment, rehabilitation, housing and supportive services, most specifically entitlements (SSI, PA, etc.)
- Escorts patients to their outpatient mental health or substance abuse treatment appointments to enhance post discharge care coordination, when necessary.
- Assists the consumer with activities of daily living, e.g. setting up a household, public transportation, budgeting, shopping, socializing, parenting, and other skills relevant to an individual's rehabilitation goals. May make home visits.
- Advocates for the needs of people with psychiatric disabilities within the mental health system and within service delivery systems.
- Responsible for Client Services Dollars.
- ALL ABOVE DUTIES MAY CHANGE AT THE DISCRETION OF THE DEPARTMENT HEAD/SUPERVISOR, ACCORDING TO THE NEEDS OF THE SERVICES.
Minimum Qualifications
Any individual working in a NYS Office of Addiction Services and Supports (OASAS)-Certified Treatment Program or newly-hired into the System in the title of Addiction Program Administrator in a NYC Health + Hospitals health care setting must have proof of completion of OASAS “Supporting Recovery with Medications for Addiction Treatment (MAT)” training.
For Assignment to Level I
1. Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) - Level 2 Certificate issued by the NYS Office of Addiction Services and Supports (OASAS); or
2. Is otherwise considered a Qualified Health Professional (QHP) based on licensure, certification and/or registration along with appropriate experience and/or training as defined in Section 800.4 (U) of the OASAS Treatment Services: General Provisions, Title 14 NYCRR Part 800 (/files/documents/2022/09/800.pdf).
Department Preferences
- The Senior Addiction Counselor will have experience working with substance abuse patients, ability to work in patients' home and the community and has knowledge of community resources.
- CASAC and NYS Driver's License are required.
