Job Description I-RISE Case Manager - Interfaith-RISE - Reformed Church of Highland Park Affordable Housing Corporation
Position Description
The Case Manager is responsible for providing comprehensive case management to refugee families, focusing on both pre-arrival and post-arrival services. This role ensures that refugee clients receive the necessary support through their first 90 days of core services, empowering them to achieve self-sufficiency.
Responsibilities
- Conduct in-home needs assessments for adult clients and collaborate with families to develop personalized service plans. Offer individualized support through direct services, referrals, and advocacy to help clients progress toward self-sufficiency.
- Continuously assess and track client progress toward established goals. Identify and address challenges, ensuring that all available resources are fully utilized for client success.
- Maintain detailed records of each client interaction according to program requirements. Input client information into databases (Apricot) and assist with tracking and reporting as needed.
- Work effectively with coworkers and agency program directors. Build and maintain strong relationships with local service providers to benefit client needs.
- Prepare / Assist with submission of reporting with program director.
- Meet timelines and deadlines related to supporting systems and employee compliance
- Assure that regular case management meetings are scheduled with clients (minimum of once per month). Conducting client check-ins as needed to assure compliance and quality service
- Serving as the first point-of-contact for client issues / complaints and reporting to program director as needed for further follow-up
- Work cooperatively with the RCHP-AHC Leasing Office to obtain needed paperwork including signed leases and renewals, and to facilitate communication about maintenance, rent payment, unit care and other topics.
- Assist housing coordinator for R&P arrivals
- Other tasks as assigned
Qualifications
Bachelors degree in social work or a related field preferred.Minimum of 2 years of relevant professional experience in the human services field required.Experience working with refugee and / or immigrant populations strongly preferred.Strong understanding of benefits and state-funded resources for refugees, along with referral pathways.Ability to work independently with strong initiative and problem-solving skills.Demonstrated success in a multicultural environment with effective communication skills.Excellent time management and multitasking abilities.High attention to detail and accuracy in documentation.Proficiency in Microsoft Office applications (Word, Excel, Outlook), as well as digital tools for data entry, research, and problem-solving.Ability to drive within Monmouth and other neighboring counties.Language Skills :
