Job description

Job Description : 📌 Role Overview :

In this role, you will assist consumers in accessing essential community services (medical, psychological, substance use, vocational, food, clothing, etc.) through referrals, advocacy, and follow-up.

🕒 Schedule : Tuesday – Saturday, 9 : 00 AM – 5 : 00 PM

💲 Pay : $24 / hr

✨ Key Responsibilities :

  • Manage caseloads and develop individualized support plans.
  • Provide supportive counseling, problem-solving, and crisis intervention.
  • Accompany consumers to appointments and assist with daily living needs.
  • Advocate for consumers and promote self-advocacy.
  • Coordinate community-based services and activities for social / skill development.
  • Document services and maintain relationships with therapeutic support providers.
  • Assist with moves and provide on-call crisis support as needed.

✅ Requirements :

  • Bachelor’s Degree.
  • Minimum 2 years of experience in behavioral health.
  • Proficiency in Microsoft Office and general computer programs.
  • 💡 Preferred Qualifications :

  • Experience with individuals experiencing mental illness, substance use, or homelessness.
  • Background in case management, community outreach, or supportive housing.
  • Bilingual in English / Spanish preferred.
