Case Manager 039 (DASH and Housing Supports) - Edison NJ Salary : $50,000 per year Key Responsibilities : Dash : Primary role is to support individuals and families who are referred through DCA experiencing homelessness and are in need of housing preparation case management. S / he will provide identification of barriers to tenancy, identification of credit and criminal histories, monthly budgeting, linkages to housing adjacent systems of care and mainstream benefits and entitlements. Housing Support : Communicating with various MCOs. Outreaching referred clients, completing intake paperwork, submitting paperwork to request Pre-Authorization, Inputting approved clients in the system (WellSky and MyEvolv). Attending training per Medicaid contract. Both programs require : Providing case management services to at-risk or homeless households in Middlesex County, includes service planning, linkage, crisis intervention, de-escalation, managing relationship with a broad range of community providers / landlords, housing retention. Requirements : Bachelor’s Degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field is preferred At least two (2) years of relevant work experience and demonstrated ability to provide effective services to clients required.
