As the Case Worker - 1 115 Waiver Screener for Catholic Charities of Community Maternity Services , you will play a pivotal role in individuals' lives, making a significant impact through :

Duties and Responsibilities - Case Manager / Parent Educator

• Assess the social, educational and medical needs of Maternity Outreach clients.
• Act as case manager and coordinate services for Maternity Outreach clients.
• Provide goal-directed counseling for Maternity Outreach clients.
• Work closely with Maternity Outreach clients to assure that needed services are provided and utilized.
• Provide instruction in child development and parent education to parents

Duties and Responsibilities - 1115 Enhanced Navigator

Case Worker - 1 115 Waiver Screener Minimum Salary : $22.00 / Hour

Case Worker - 1 115 Waiver Screener Maximum Salary : $22.00 / Hour

Case Worker - 1 115 Waiver Screener Schedule : Monday - Friday 8 : 30AM - 4 : 30PM with flexibility to meet client needs (35 hours per week)

Qualifications :

Associates Degree in Human Services or related field

1-3 years of relevant experience
