As the Case Worker - 1115 Waiver Screener for Catholic Charities of Community Maternity Services, you will play a pivotal role in individuals' lives, making a significant impact through :
Duties and Responsibilities – Case Manager / Parent Educator
Assess the social, educational and medical needs of Maternity Outreach clients.
Act as case manager and coordinate services for Maternity Outreach clients.
Provide goal-directed counseling for Maternity Outreach clients.
Work closely with Maternity Outreach clients to assure that needed services are provided and utilized.
Provide instruction in child development and parent education to parents
Duties and Responsibilities – 1115 Enhanced Navigator
Conduct screenings with Medicaid eligible individuals for services under the 1115 Waiver using the approved ACH screening. Ensure all demographic information is accurately collected and member consent is properly documented.
Deliver basic navigation services to eligible individuals, including eligibility assessments, resource referrals, and documentation of all services provided in the Unite Us platform.
Provide enhanced navigation / care management services to households determined eligible, including development of Social Care Plans, assistance with applications, care coordination, referral follow-up, and comprehensive case documentation in Unite Us.

Case Worker - 1115 Waiver Screener Maximum Salary : $22.00 / Hour
Case Worker - 1115 Waiver Screener Schedule : Monday – Friday 8 : 30AM – 4 : 30PM with flexibility to meet client needs (35 hours per week)
Qualifications :
Associates Degree in Human Services or related field
1-3 years of relevant experience
