Responsibilities:
Assess and treat patients with a wide range of physical therapy needs, including orthopedic, neurological, and cardiovascular conditions
Develop and implement individualized treatment plans to promote optimal patient outcomes
Collaborate with interdisciplinary teams to ensure comprehensive care and coordination
Communicate effectively with patients, families, and healthcare professionals to ensure seamless transitions and ongoing care
Document patient progress and maintain accurate records
