About The Position

Job Title : Acute Care Social Worker

Profession : Behavioral Health

Specialty : Medical – Discharge Planning

Duration : 13 weeks

Shift : Days with rotating weekends

Hours per Shift : 36-40 hours per week

Experience : Minimum of 3 years of clinical experience in a hospital setting

License : State licensure as a Licensed Clinical Social Worker (LCSW)

Certifications : Master's degree in Social Work

Must-Have : Discharge planning experience

Description :

This Traveler Assignment will involve working alongside RN Case Managers.

The position may include working in the inpatient or emergency department.

The role involves assessing patients and / or patients' families, caregivers, or legal representatives and arranging for necessary interventions.

Care-focused interventions are planned to help patients cope with physical, social, emotional, economic, and environmental concerns.

Treatment planning for support will also be provided.

Psychosocial support will be offered to patients through direct counseling and collaborative visits, with cultural humility and sensitivity.

Care psychosocial assessments will be performed and documented.

Collaboration and resource-sharing with social worker team members working with patients in the hospital will be integral to the role.

Participation in the resourcing of outpatient services relevant to patients is expected.

Coordination of educational and outreach services is also part of the responsibilities.

Critical thinking skills, decisive judgment, and the ability to work with minimal supervision are essential.

The candidate must be able to thrive in a fast-paced environment and take appropriate action when needed.

Flu vaccine is required with no exemptions.
