TAG MedStaffing is hiring a Behavioral Health Clinician (Integrated Care) in Burton, MI, for our client. This full-time, in-office role supports patient care through behavioral health interventions, crisis assessments, and coordinated care planning within a primary care setting. Ideal for master's-level clinicians passionate about integrated care, mental health, and improving access for underserved populations.

Responsibilities as the Behavioral Health Clinician (Integrated Care) :

• Clinical Interventions : Provide brief behavioral health services, including short-term counseling, psychoeducation, crisis support, and substance use interventions.
• Integrated Care Collaboration : Work closely with medical providers and care teams to address behavioral and social determinants of health within a coordinated care model.
• Assessment & Referral : Conduct behavioral health and substance use assessments, and facilitate referrals to internal or community-based resources.
• Patient Advocacy : Support patients in overcoming social, environmental, and socioeconomic barriers to improve health outcomes.
• Documentation : Maintain timely, accurate clinical records in compliance with organizational and regulatory standards.
