Family Practice / Primary Care Nurse Practitioner

Outstanding primary care group in the Northern California area is seeking an experienced nurse practitioner to join their growing team!

This position is a mainly clinic-based role, with a small portion of in-home duties within the local community. You will deliver high-touch care to geriatric patients with the goal of preventing hospital readmissions, as well as managing chronic conditions and coordinating care across specialties.

Ideal candidates will have experience working as a nurse practitioner in primary care, family medicine, internal medicine, gerontology, or another relevant field.

Full-time permanent position - Primary Care

Monday-Friday schedule

Focus on geriatric high-risk patients

See patients mostly in clinic (about 75%) and some in-home visits (about 25%)

No telehealth

Work alongside a robust support team of medical assistants, MD advisor, and clinical leadership

Deliver high-touch care to geriatric patients with complex needs

Prevent hospital readmissions and reduce avoidable utilization

Conduct AWVs, risk assessments, medication management

Manage chronic conditions and coordinate care across specialties

Participate in IDT meetings and quality initiatives

Requirements :

Experience as a nurse practitioner in primary care, internal medicine, gerontology, or other relevant fields

California Nurse Practitioner license

Spanish language skills (preferred)
