Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.
An opportunity is available for a skilled Home Health Registered Nurse (RN) to deliver compassionate, patient-centered care in a home setting. This role requires an experienced nurse capable of conducting thorough assessments, managing individualized care plans, and collaborating with interdisciplinary teams to support patient well-being in their own homes.
Responsibilities :
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate effectively with care teams and physicians.
- Educate patients and families about disease processes, treatment regimens, medications, and preventive measures.
- Maintain timely and accurate electronic documentation using Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and home health aides to ensure coordinated care.
- Participate in case conferences, staff meetings, and ongoing education.
- Adhere to regulatory and agency standards for documentation and patient care.
Qualifications :
Active RN license in good standing for the practicing state.Minimum one year of recent home health experience; field experience preferred.Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool.Strong clinical assessment abilities and critical thinking skills.Reliable transportation with valid drivers license and auto insurance.Ability to work independently and manage a flexible, field-based schedule.Excellent communication, time management, and organization skills.Bachelor of Science in Nursing (BSN) preferred.Familiarity with Medicare and home health compliance standards is a plus.Location : This position is based in Fayetteville, North Carolina, a vibrant city known for its rich history, growing healthcare community, and welcoming environment. Contract Details :
