The Opportunity
In partnership with the Independent Drivers Guild (IDG) and the Workers Benefit Fund (WBF) , Akido is launching an innovative health program for New York City's independent drivers — the people who keep the city moving but are often left out of traditional healthcare systems.
We're seeking a Comprehensive Care Coordinator who is passionate about community health, program coordination, and relationship-based care. This role blends direct patient engagement with program design and operations, helping to build a model of whole-person care that goes beyond the exam room.
This is not a typical care coordinator role. You'll work alongside a small, collaborative care team — including a physician, nurse practitioner, dietitian, and health coach — to help patients navigate care, build healthy habits, and access resources that address physical, mental, and social well-being.

What You'll Do Care Coordination & Patient Support
Serve as the primary point of contact for participants enrolled in Akido's Worker Healthy Living Program.
Build trusting relationships through consistent outreach, follow-up, and encouragement.
Support patients in navigating care plans, scheduling visits, and accessing specialty or community-based resources.
Coordinate across the care team to ensure seamless, integrated care delivery.
Track patient progress using Akido's digital tools and dashboards, ensuring timely follow-up and documentation in the EMR.
Identify and address barriers to care — including insurance navigation, social needs, and health literacy challenges.
Program Implementation & Operations
Support the delivery of group visits and wellness programs focused on nutrition, movement, sleep, and stress management.
Coordinate logistics such as session scheduling, participant reminders, and follow-up outreach.
Partner with clinical and operational leaders to refine workflows that make care more accessible and efficient.
Gather feedback from patients and staff to improve the program experience and outcomes.
Assist in data collection and reporting to support quality improvement and performance monitoring.
Community Engagement
Represent Akido's program at driver hubs, community events, and outreach activations across NYC's five boroughs.
Build local partnerships with organizations that support health, wellness, and driver well-being.
Help design and implement engagement strategies that foster belonging and empower participants to take charge of their health.
Who You Are
Passionate about health equity, prevention, and person-centered care .

Comfortable in both field-based and virtual settings.
Energized by collaboration and continuous improvement.
Required Qualifications
3+ years of experience in care coordination, program coordination case management, community health, or health program operations.
Demonstrated ability to work effectively with underserved or marginalized populations.
Strong communication and interpersonal skills.
Proficiency with EMRs or digital health documentation systems.
Experience coordinating care across multiple disciplines.
Preferred Qualifications
Background in community or public health, lifestyle medicine, or chronic disease management.
Bilingual
Familiarity with NYC community resources, benefits programs, and social services navigation.
Experience supporting mobile or hybrid care models.
