A company is looking for a Clinical Pharmacist Market Support Clinical Excellence Team.

Key Responsibilities

Conduct telephonic patient consultations and provider-facing activities to improve STARs measures

Develop market business plans and provide analytical interpretation of STARs and HEDIS reporting

Complete annual medication reviews and pharmacotherapy assessments for patients

Required Qualifications

Doctor of Pharmacy degree or equivalent experience

Active, unrestricted pharmacy license in primary residence and Oregon

5+ years of Clinical Pharmacy Practice experience

2+ years of experience with Medicare STARs & HEDIS Measures

2+ years of experience in provider-facing interactions and data analysis
