Description

This part time (32 hours / week) position will be working variable days and evenings. This role will be traveling to see hospice patients primarily in East and West Portland areas.

The Hospice Access Field and Triage RN will complete medical requirements while coordinating a whole-person plan of care. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. In Field role, the RN will visit patients in their homes (private or facility) to present the hospice program and conduct eligibility assessments, coordinating care with Hospice MD, family and facility caregivers, and interdisciplinary team (IDT).

Required Qualifications :

  • Education sufficient for licensure
  • Upon hire : Oregon Registered Nurse License
  • Within 6 months (180 days) of hire : Washington Registered Nurse License
  • Upon hire : National Provider BLS - American Heart Association
  • Upon hire : In compliance with the Providence Health and Services-OR, criminal background check is required for this position, based on assigned duties
  • 2 years Acute care and / or hospice, home health, or long-term care RN experience.

Preferred Qualifications :

  • Bachelor's Degree in Nursing
  • Call Center experience.
