Overview

Parent Coach / CHW - Primary Care (Whitehall PCC)

The Parent Coach develops, implements, and delivers effective and family-centered preventive care services and counseling to families at well-childcare visits for children ages 0 to 3.

The Parent Coach functions as a liaison between the patient, their family, their community, and the Primary Care clinical teams.

Responsibilities

  • Provides skilled evaluations of family needs for supportive services, a wide spectrum of information, education, and referrals to parents on preventive, developmental, and behavioral topics, as well as parenting support in accordance with the Bright Futures Guidelines.
  • Assists with preventative services for children up to the age of 18 if no children aged 0 to 3 years are present. Provides education and links to community resources.
  • Facilitates communication between providers, family, and other resource service providers. Participates in training and precepted visits.
  • Maintains accurate patient and program documentation.
  • Develops, creates, and standardizes health education materials that are culturally and linguistically appropriate.
  • Leads in the implementation of evidence-driven best practices in health education and outreach.
  • Functions as a member of the multi-disciplinary team to facilitate project objectives. Participates in weekly project meetings.

Qualifications

KNOWLEDGE, SKILLS AND ABILITIES

Education Requirement :

  • High School Graduate or equivalent, required.
  • Associate or bachelor’s degree, preferred.

Certifications :

  • Community Health Worker, preferred.
  • Basic Life Support, preferred.
  • National Passenger Safety Car Seat Certification, preferred.

Skills :

  • Effective customer service, communication, and interpersonal skills required for interaction with patients, families, and community providers.
  • Ability to work as a part of a team, with diverse populations, (i.e. racial / ethnic, socioeconomic and educational backgrounds), and with healthcare providers in clinical settings.
  • Extensive knowledge of maternal and child health issues.
  • Ability to work as a part of a team and with health care providers in clinical settings.

Experience :

Experience working or volunteering with children and families, required.
