Social Worker BSW

Position Summary

The Social Worker (SW) serves as a coordinator of an interdisciplinary approach to guide patients through the continuum of care with a special focus on discharge planning, psycho-social needs assessment, care / resource management, and provider / insurance issues. Collaborates with the physicians, patients, their family members, and others to facilitate the achievement of desired outcomes, address ongoing needs, maximize patient, family, and provider satisfaction, optimize systems / resource utilization, facilitate timely discharge, assure appropriate length of stay, reduce unnecessary delays and other adverse variances to the delivery of services.

The Social Worker's decisions and actions with patients and families are determined in an ethical manner practicing compassion and respect for the inherent dignity, worth, and unique attributes of each person. Effectively communicates with patients / families, visitors, colleagues, physicians, leadership, and volunteers. All areas of practice and care delivery are provided to achieve and maintain top-decile results and outcomes for patient experience, quality, and patient safety. The Social Worker follows and adheres to UNC Health Appalachian's policies and procedures, regulatory requirements, and standards of behavior. The Social Worker practices within the practice and scope of their license.

Always assists coworkers, patients, families, and visitors regardless of the patient assignment, so long as the assistance is in-line with their scope of practice and license.

Compensation is commensurate upon education, experience, certification, and internal equity. Hiring compensation range starts from $26.54 / hour.

• Bachelor's degree in Social Work from an accredited college or university.
