This position requires working two weekends every six weeks.

The candidate must possess the ability to organize and participate in family meetings.

Participation in the development of treatment plans is necessary.

Experience with health maintenance organizations, independent practice associations, and the ability to arrange durable medical equipment, skilled nursing facilities, and ambulance transport independently is required.

The candidate must be proficient in computerized documentation.

Interdisciplinary support is available, including physical therapy and respiratory services.

Knowledge of various patient diagnoses is advantageous.

Compliance with health regulations is essential.

The candidate must be able to determine medical necessity per evidence-based guidelines.

There will be an expectation to float between facilities as needed.

Great communication skills are vital for collaboration with various teams and departments.

The role includes coordinating care and managing discharge planning effectively.

Commitment to holiday coverage is essential.

References will be required from previous supervisory roles.
