What is a Home Health Care Hospital Liaison?

The Hospital Liaison enhances and promotes Concierge Home Care's mission, vision, and values in the community by bridging the gap between a healthcare system and a patient’s safe home arrival.

What does a Home Health Care Hospital Liaison do?

The Hospital Liaison is responsible for developing and creating outreach and education programs to predominantly Hospital healthcare professionals, facilities, organizations, and other healthcare consumers to provide quality, safe, and cost-effective post-hospital nursing and therapy home healthcare services to seniors.

What are some functions of the Home Health Care Hospital Liaison?

• Establishes grows, and maintains customer relationships to increase and maintain sales revenue of Concierge Home Care’s post-hospital care services
• Maintains in-depth knowledge of the community, healthcare facilities, and referral sources appropriate for post-hospital
• Develops and maintains professional relationships with executive-level leadership, physicians, nurse practitioners, physician assistants, discharge planners, case managers, social workers, and other health care professionals involved in home health care coordination, by regularly conducting visits to referral sources to assist in identifying appropriate patients that may benefit from home health care services specific to post-hospital care needs
• Facilitates care coordination and collaboration of patients’ care between Concierge Home Care and referral sources

What are the qualifications for a Hospital Liaison?

• Bachelor’s Degree or demonstrated experience that serves as equivalent
• Home health or health care background preferred
• A clinical background is required for this position is required (RN, LPN, MSW)
• Ability to communicate effectively with clients, coworkers, and other healthcare professionals
• Excellent organizational and communication skills
