This full-time role offers a flexible 4-5 day workweek. Role Highlights: Clinical Care: Conduct primary care rounds, manage chronic diseases, and implement complex care plans in post-acute settings. Leadership: Provide oversight and mentorship to Nurse Practitioners. Quality & Collaboration: Lead clinical case reviews, collaborate with facility leadership, and develop care coordination initiatives. Relationship Building: Foster strong relationships with patients, families, and the post-acute care community. Qualifications: Minimum of one year of primary care experience required.
