Responsibilities Conduct Comprehensive Patient Assessments : Perform initial and ongoing evaluations of patients to assess physical capabilities, limitations, and functional goals, taking into consideration medical history, diagnoses, and current condition. Develop and Implement Treatment Plans : Create personalized, evidence-based treatment plans tailored to each patients needs, focusing on improving mobility, strength, and balance. Therapeutic Interventions : Apply a variety of therapeutic modalities and exercises to address patient conditions, such as strength training, balance exercises, mobility improvement, and pain management techniques. Patient Education and Family Support : Provide education to patients and their families about the treatment process, home exercise programs, and safety protocols. Offer strategies to manage pain, prevent falls, and promote self-care outside of scheduled therapy visits. Monitor and Adjust Treatment Plans : Continuously evaluate patient progress, modifying treatment plans as needed to ensure the best possible outcomes. Collaborate with patients, families, and the healthcare team to make adjustments to care as necessary. Documentation and Compliance : Maintain detailed and accurate records of all patient care activities, treatment interventions, and progress notes. Ensure all documentation complies with HIPAA and state regulations. Team Collaboration : Work closely with other healthcare professionals, including physicians, nurses, occupational therapists, and social workers, to provide holistic care and ensure patients needs are met. Continuing Education : Stay up to date with the latest physical therapy techniques, industry standards, and best practices by participating in ongoing professional development opportunities. Experience and Qualifications Educational Requirements : A degree in Physical Therapy from an accredited institution. Licensure : Must hold a valid state licensure as a Physical Therapist in California, or be eligible for licensure. CPR Certification : Current CPR and BLS certification required. Experience in Home Health : Previous experience in home health is preferred, but not mandatory. Candidates with strong clinical backgrounds in other settings are encouraged to apply.
