Responsibilities:
Develop and implement effective social services programs and services that meet the needs of our diverse resident population
Collaborate with the interdisciplinary team to ensure seamless care coordination and delivery
Provide emotional support and counseling to residents and family members
Develop and maintain relationships with community resources, organizations, and referral sources
Recruit, train, and supervise social services staff and volunteers
Participate in quality improvement initiatives to ensure continuous improvement and excellence in social services
Maintain accurate and timely records and reports
