MDS Coordinator

A Nursing home is seeking a detail-oriented and experienced MDS Coordinator to join their clinical team. This role is ideal for a nurse who is strong in assessment, documentation, and care planning—and who understands how to drive accurate reimbursement and quality outcomes.

Key Responsibilities

• Complete and manage the full RAI/MDS process in accordance with federal and state guidelines.

• Conduct resident assessments and develop individualized care plans.

• Collaborate with nursing, therapy, social services, and dietary teams.

• Ensure compliance with PDPM, CMS regulations, and facility policies.

• Monitor deadlines, coding accuracy, and documentation requirements.

• Participate in quality improvement initiatives, care plan meetings, and audits.

• Provide education to staff on MDS processes as needed.

Qualifications

• Active RN license in PA.

• Prior MDS/RAI experience in a Skilled Nursing Facility required.

• Strong knowledge of PDPM, care planning, and CMS regulations.

• Excellent documentation, analytical, and communication skills.

• Highly organized with strong attention to detail.

• Ability to collaborate effectively with interdisciplinary teams.
