The Opportunity:

Overview:

This is a Limited duration position. Limited duration positions can last up to two (2) years, but can end at any time.

As the Case Manager Senior, other duties will include:

Advanced Case Management and Risk Mitigation

  • Conduct comprehensive, advanced assessments to determine the complex and often multi-system support needs of children identified by the preschool provider and preschool for all inclusion coordinator.

  • Develop, implement, and monitor a support plan that addresses the unique needs, choices, strengths, and risks of the child to successfully access preschool.

  • Monitor and document the use of behavior services to ensure compliance with Department approved behavior model, Positive Behavior Support (PBS), Oregon Intervention System (OIS) and OARs regarding psychotropic medications.

  • Develop and implement protocols with partner agencies for the identification of appropriate high-risk individuals, development of collaborative care plans, and effective service delivery.

  • Identify gaps in available community services and make data-driven recommendations for cost-effective solutions.

Service Coordination and Monitoring

  • Assess and facilitate services to assist assigned children in transitioning to an educational setting

  • Provide technical assistance to service coordinators (case manager 2 classification) and their supervisors to approve appropriate resources to access preschool.

  • Monitor all services and supports via visits to preschool providers sites, high frequency of contact with assigned children’s parents and other community partners to assist children to successfully access preschool.

  • Ensure that all resources are used appropriately to meet the client's developmental needs.

Documentation and Compliance

  • Maintain accurate case records and document all actions taken in accordance with federal, state (OARs), and local policy.

  • Assess which program will be identified as the funding resource for assigned children’s services.

  • Develop, submit, and modify necessary documentation promptly to implement, change, or adjust payments and program benefits.

Training, Technical Assistance and Program Guidance

  • Provide technical assistance and training to individuals, families, care providers, and community partners on the implementation of Support Plans (ISPs), service delivery protocols, and risk mitigation strategies.

  • Conduct training sessions or provide one-on-one technical support regarding services to preschool providers

  • Participate in agency meetings and case conferences to ensure alignment on care planning, risk assessment, and service integration in preschool settings.

Minimum Qualifications/Transferable Skills*:

A case manager must have knowledge of the public service system for developmental disabilities services in Oregon and at least one of the following:

  • A bachelor’s degree in behavioral science, social science, or a closely related field.

  • A bachelor’s degree in any field and one year of Human services related experience, such as work providing assistance to people and groups with economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing.

  • An associate’s degree in a behavioral science, social science, or a closely related field and two (2) years of human services related experience such as work providing assistance to people and groups with economical disadvantages, employment, abuse and neglect, substance abuse, aging disabilities, prevention, health, cultural competencies, or housing.

  • Three (3) years of human services related experience, such as work providing assistance to people and groups with economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention of health, cultural competencies, or housing.

  • Must have a valid driver license
