What You’ll Do as a Hospice Nurse Practitioner

In this role, you’ll provide expert hospice and palliative care to patients and families while ensuring comfort, dignity, and peace during life’s most meaningful journey.

You will :

Perform routine and emergency face-to-face recertification visits for patients entering their 3rd benefit period or beyond, including admissions, transfers, and extended stays.

Assess, document, and communicate clinical findings with the Medical Director, Attending Physician, and interdisciplinary team (IDG).

Provide physical and emotional support to patients and families, especially during crisis situations.

Triage consults and prioritize care based on patient acuity.

Deliver bedside palliative care and education for patients and caregivers.

Prevent unnecessary hospitalizations by meeting patient needs at home .

Maintain accurate and compliant documentation , billing, and coding per CMS guidelines.

Support quality care through staff training, continuing education , and clinical guideline development.

What You Bring to the Team

Master’s Degree in Nursing and national certification as a Nurse Practitioner

Active state NP license and ability to maintain licensure

1+ year of clinical nursing experience (hospice or palliative preferred)

Experience in Family Medicine, Geriatrics, or Psychiatry is a plus

DEA certification (if applicable for prescribing in your state)

Valid driver’s license, insurance, and reliable transportation

Ability to travel up to 50% , and lift / move up to 50–100 lbs as needed
