Position Summary

Healthonomic Primary Care is seeking a full-time Family Nurse Practitioner (FNP) to join our outpatient practice in Orland Park, Illinois. The Nurse Practitioner will provide comprehensive, evidence-based primary care services to individuals and families in collaboration with our team of physicians. This position offers the opportunity to enhance patient care, improve healthcare outcomes, and optimize clinical workflows within a supportive environment.

Responsibilities

• Provide comprehensive primary care services across the lifespan, including health promotion, disease prevention, diagnosis, and management of acute and chronic conditions.
• Support initiatives to enhance clinical integration metrics.
• Engage with patients to address concerns and foster positive relationships. Serve as a liaison between patients and the healthcare team to ensure seamless communication.
• Maintain clinical documentation in accordance with applicable laws and practice standards; close charts within 48 hours of each patient encounter.
• Ensure compliance with internal policies, billing protocols, HIPAA standards, and regulatory guidelines.
• Promote the practices services through community engagement such as lectures, health fairs, and educational events.
