Employment Type: Per Diem – Field Visits local to Manchester, NH
We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our Greater Manchester home care team on a per diem basis. This position offers independence, flexibility, and meaningful one-on-one care for patients who prefer to remain in the comfort of their homes throughout Manchester and surrounding communities.
The LPN will provide skilled nursing visits focused on promoting recovery, maintaining stability, and enhancing quality of life. This is an ideal role for nurses who thrive in autonomous settings and want flexible scheduling with competitive per-visit compensation.
• Conduct in-home patient assessments, medication management, and wound care.
• Monitor and document changes in patient condition and report to supervising RNs or physicians.
• Educate patients and caregivers on treatment plans, medications, and home safety practices.
• Maintain accurate, timely documentation using electronic charting.
• Collaborate with multidisciplinary care teams to ensure coordinated and effective patient care.
• Competitive per-visit pay, averaging $50+/hour based on actual time spent per visit.

• Current and active LPN license in the State of New Hampshire (required; not a compact state).
• Valid driver’s license and reliable transportation.
• Prior experience in home care, long-term care, rehabilitation, or community nursing preferred.
