Key Responsibilities of the Physical Therapist

• Conduct SOC (Start of Care), evaluations, reassessments, and discharges
• Coordinate visit times with patients and manage daily schedules
• Collaborate with PTAs for routine visits
• Complete documentation in partner agency EMRs, including Pariox, Kantime, Kinnser (WellSky), Axxess, and Homecare Homebase

Requirements of the Physical Therapist

• Valid Physical Therapy license
• Experience in home health settings preferred
• Proficiency with EMR systems
• Ability to cover Saturday orthopedic visits
