Position Summary

Works in accordance with the plans and policies established by the Board of Directors within the health care setting, which includes mental health and as a member of the Service Team, providing direct therapy services to clients of the Center. This employee will function independently and autonomously. Work is to be performed under general supervision and administrative direction and will be review through conferences and reports for achievement of the desired results.

Essential Duties & Functions

• Provide cognitive and behavioral interventions to individuals (adult and pediatric patients) with a behavioral health problem
• Provide classes in a format appropriate for a primary care setting (i.e., stress management, sleep enhancement and chronic pain)
• Consult with primary care physicians (PCP) regarding patient care, as requested
• Timely follow-up on all PCP referrals via same-day verbal and / or written communication; timely documentation in the patients’ medical record
• Deliver brief presentations / trainings to primary care team members regarding behavioral health related issues and BHC services provided
• Participate in the development, implementation, evaluation, and revision of clinical pathway programs
• Consult with legal authorities, as directed
• Support the Mission, Vision and Values of TFHC
• Effectively manage a full daily patient schedule as defined by the BHC manual
• May include supervision of peer providers who are in training or not yet fully licensed
• Attend and participate in meetings, committees, and conferences, as directed
