The Medical Group provides a wide range of offerings, including primary care, specialty care, behavioral and mental health, and urgent care, both digitally as well as at physical locations in Massachusetts, New Hampshire, and Maine. The group also offers outpatient surgery and endoscopy, imaging, cardiac testing, and infusion. We share the commitment to delivering a coordinated and comprehensive experience across all locations, ensuring the appropriate level of care is available to every patient across our care delivery sites.We are seeking a part-time, 32-hour Diabetes Educator to join our outpatient Diabetes Services team. This position will provide support across multiple locations, with one day per week based in Rochester and three days per week in Dover, collaborating closely with primary care and internal medicine teams.The ideal candidate is a Registered Nurse + Certified Diabetes Educator (CDE) or Registered Dietitian + Certified Diabetes Educator (CDE) with a passion for patient education and chronic disease management. This role will work alongside and support another Diabetes Educator while partnering with providers and care teams to deliver comprehensive diabetes education and support to patients across the continuum of care.Responsible for providing comprehensive education and support to patients with diabetes, promoting self-management skills, and enhancing health outcomes. This role involves working collaboratively with healthcare teams to develop individualized care plans and facilitate diabetes management programs.Does this position require Patient Care? YesEssential Functions:Conduct assessments to determine the educational needs and readiness of patients to learn about diabetes management.Develop and implement individualized education plans addressing diabetes self-management, including medication management, nutrition, physical activity, and blood glucose monitoring.Facilitate group education classes on diabetes management, prevention, and lifestyle modifications.Create educational materials and resources to enhance patient understanding and engagement.Provide ongoing support and counseling to patients and their families, addressing the emotional and psychological aspects of living with diabetes.Encourage patient adherence to treatment plans and empower them to actively participate in their health.QualificationsEducationAssociate's Degree Related Field of Study requiredBachelor's Degree Related Field of Study preferredMaster's Degree Related Field of Study preferredCan this role accept experience in lieu of a degree?NoLicenses and CredentialsCertified Diabetes Educator [CDE/CDCES] - Certification Board for Diabetes Care and Education requiredExperienceExperience in health and nutrition related to diabetes education, counseling, or related role 2-3 years requiredKnowledge, Skills and AbilitiesStrong knowledge of diabetes pathophysiology, management strategies, and treatment options.Excellent communication and interpersonal skills to effectively educate and support patients and families.Ability to work collaboratively within a multidisciplinary healthcare team.Proficiency in using educational tools and technology to enhance patient learning.Additional Job Details (if applicable)The Diabetes Educator will play a key role in empowering patients through education, clinical guidance, and ongoing support, working closely with providers and caregivers to improve long‑term health outcomes.
