Experienced Chronic Care Manager Supervisor - Vitability Health New Jersey

Job Overview:

We are seeking an experienced and dedicated professional to join our healthcare team as a Chronic Care Manager Supervisor. The ideal candidate will have a minimum of 3 years of hands-on experience in chronic care management, coupled with at least 1 year of supervisory experience. In this role, you will be responsible for overseeing and guiding a team of chronic care managers to ensure the delivery of high-quality, patient-centered care for individuals with chronic health conditions.

Responsibilities:

  • Supervision and Leadership:

    • Provide leadership and guidance to a team of chronic care managers.

    • Conduct regular team meetings to facilitate open communication, share insights, and address challenges.

    • Foster a culture of collaboration, continuous learning, and excellence within the team.

  • Chronic Care Management:

    • Utilize your expertise in chronic care management to support the development and implementation of care plans.

    • Oversee the development and implementation of individualized care plans for patients with chronic conditions.
    • Ensure that care plans are tailored to meet the unique needs of individuals with chronic health conditions.

    • Proactively identify and address barriers to effective care coordination.
    • Stay abreast of the latest developments in chronic care management and incorporate best practices into team processes.

  • Quality Assurance:

    • Monitor and evaluate the quality of care provided by the chronic care management team.

    • Implement and oversee quality improvement initiatives to enhance patient outcomes.

    • Ensure compliance with relevant healthcare regulations and standards.

  • Team Training and Development:

    • Conduct ongoing training sessions to keep the team updated on the latest advancements in chronic care management.

    • Identify and address training needs to enhance the skills and knowledge of team members.

  • Collaboration:

    • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive and coordinated care.

    • Establish and maintain effective communication channels with internal and external stakeholders.

  • Data Management:

    • Oversee the collection and analysis of data related to chronic care outcomes.

    • Use data-driven insights to identify areas for improvement and implement changes as needed.

Qualifications:

  • Bachelor's degree in nursing, healthcare management, or a related field; Master's degree preferred.

  • Minimum of 3 years of direct experience in chronic care management (CCM), with a proven track record of positive patient outcomes.

  • At least 1 year of supervisory or managerial experience in a healthcare setting (CCM supervisor preferred).

  • Strong understanding of chronic health conditions and evidence-based interventions.

  • Excellent communication, leadership, and interpersonal skills.

  • In-depth knowledge of chronic care management protocols, best practices, and emerging trends.
  • Ability to lead a team effectively, foster collaboration, and create a positive work environment.
  • Familiarity with healthcare regulations and compliance requirements.
