Position: Home Care Clinical Coordinator
Location:

Type: Full-Time
Key Responsibilities:
Maintain and organize client files
Monitor file compliance and ensure all required documentation is current
Track client assessments, reassessments, care plans, and supervisory visits
Coordinate with staff to obtain missing documentation and required updates
Assist with state surveys, audits, and compliance reviews
Review records for completeness and accuracy
Maintain electronic and paper records in accordance with company policies
Support the office team with administrative and compliance-related tasks
Communicate with clients, caregivers, and healthcare professionals regarding required documentation
What We're Looking For:
Experience:
2+ years of home care, healthcare administration, compliance, or related experience preferred
Education:
High school diploma required
Healthcare administration or clinical background preferred
Skills:
Strong organizational and communication skills
Excellent attention to detail
Ability to manage multiple deadlines and priorities
Computer proficiency and experience with electronic record systems
