Position Summary

The Youth Healthcare Academy Program Coordinator Assistant is a dynamic and dedicated member of the Hometown Healthcare Academy team who will coordinate the responsibilities in the youth as change agents programming including, but not limited to, the "Hometown Healthcare Academy" (HHA) healthcare pathway program and other community engaged, youth development programming. Reporting into the Program Manager this role is pivotal in enhancing the well-being of youth and supporting them in becoming active healthcare change agents and professionals in the community. Rooted in racial equity, social justice, public health and community responsiveness, this Program Coordinator will work closely with key internal and external stakeholders to implement the HHA program.

The Assistant to the Youth Health Program Manager will provide administrative and operational support to ensure the efficient and effective delivery of youth health programs. This role involves coordinating program activities, assisting with outreach efforts, tracking program metrics, and supporting the overall mission of promoting the health and well-being of youth.

Principal Duties and Responsibilities

Program Support

  • Cultivate and maintain professional relationships with youth participants, community organizations and partners.
  • Work closely with key community leaders, schools and other organizations to build strong partnerships and support for the program.
  • Purchase, pick up, transport, and distribute program supplies, educational materials, and food for youth activities, workshops, and community events.
  • Maintain an inventory of program supplies and notify the Program Manager when materials or food items need to be replenished.
  • Contribute to the planning, and implementation of the Atrius Foundation funded "Youth as Health Care Change Agents" (HHA) grant program, known as "Hometown Healthcare Academy"
  • Collaborate with HCP and CVA Coordinators to ensure engagement of families and other caregiving stakeholders to promote program participation and enhance impact.
  • Support the HHA Program Manager and collaborate with the HCP Program Coordinator to design and execute training programs, workshops and activities that foster general, youth health, health care education and leadership and advocacy education.
  • Assist in coordinating daily operations of youth health programs and activities.
  • Support the planning, implementation, and evaluation of workshops, health education sessions, and community events.
  • Monitor program timelines and help ensure activities are completed on schedule.
  • Assist with participant recruitment, registration, and follow-up.

Administrative Support

  • Maintain accurate program records, databases, attendance logs, and reports.
  • Schedule meetings, prepare agendas, and document meeting minutes.
  • Assist with correspondence, filing, and document management.
  • Support grant reporting and data collection as assigned.

Youth Engagement

  • Build positive relationships with youth participants and families.
  • Provide information and referrals to health and community resources.
  • Encourage youth participation and maintain a welcoming, inclusive environment.
  • Assist with youth leadership and peer engagement initiatives.
  • Collaborate regularly with the HHA Program Manager and Program Coordinator from partner sites to accomplish the goals and objectives of the grant.
  • Coordinate program activities, ensuring they center the youth population served and meet grant requirements.
  • Collaborates with, co-creates, coordinates and helps to lead the various elements of the program (YACH, General Health Education Sessions and Career Track education).
  • Support the successful creation of a youth advisory council (YAC) that will engage youth in the design and implementation of HHA's programming.

  • Passionate about youth empowerment and community engagement.
  • Must exercise excellent judgment in representing BNHC as this role will engage with members of the Brockton Community on vital and sensitive issues.
  • Ability to communicate effectively with colleagues, collaborators, students, caregivers and other critical stakeholders.
  • Ability to work independently, solve problems and move ahead without constant instructions
  • Ability to prioritize and manage competing demands
  • Ability to develop and work with a multi-disciplinary team, including providers, social workers, community health workers, school staff and community partners.
  • Strong computer skills including command of Word, Excel, and any other database software(s) used to track work outputs.
  • Excellent verbal and written communication skills.
  • Previous experience within a FQHC or non-profit preferred.
  • Demonstrated expertise in eliciting and managing the collaborative participation of various stakeholders
  • Excellent interpersonal skills, ability to work in a variety of settings with culturally diverse people and communities, with the ability to be culturally sensitive and appropriate.
  • Experience facilitating meetings and discussions with diverse groups of community stakeholders and other service providers.
  • Fluency in Spanish, Portuguese, French, or Haitian Creole is highly desirable. Previous experience in ambulatory care and/or public health preferred.
  • Use and understanding of the importance of creating and fostering a psychologically safe, equitable and data-driven environment.

Qualifications

  • Lived and/or work experience with Black, Indigenous, and/or People of Color (BIPOC) populations.
  • Healthcare delivery or management experience in an urban environment serving diverse, low-income populations experiencing complex social, psychosocial and medical challenges.
  • Understanding of how systemic racism, white supremacy and other forms of bias affect students and their caregivers regarding healthcare and academic engagement, retention and outcomes.
  • Sensitivity to and experience working with LGBTQ+ individuals.
  • Experience in Public Health, Social Work, Education, or related field preferred.
  • Experience working with youth programs or health initiatives is a plus.
  • Excellent verbal and written communication skills.
  • Intermediate to advanced level with Microsoft Office applications (Excel, PowerPoint and Word.)
  • Bachelor's degree or Certificate in social work, Mental Health Counseling, Nursing, Public Health or other similar healthcare field preferred. However, 3 years or more of work in Community Health, particularly working with youth, will also be considered.

Working Conditions

Works as part of a Federally Qualified Health Center. Long hours and weekend work may sometimes be required. Interruptions are present and can be disruptive to workflow. Stress is present due to a high volume of work, diverse nature of activities, and frequent crises and deadlines.

Physical Requirements

Physical demands require lifting of small equipment and/or boxes not to exceed 20 pounds. Standing, walking and sitting involved with position. Visual acuity sufficient for frequent reading and hearing acuity sufficient for holding conversations with or without the use of audio devices.
