Location: Regional Territory (Key Largo FL) travel to 810 post-acute/long-term care facilities
Job Type: Full-Time
Were hiring a Licensed Clinical Social Worker to lead behavioral health intakes and care coordination across a regional network of 810 post-acute and long-term care facilities. Youll conduct assessments evaluate patients for behavioral health and Value-Based Care programs and support the clinical development of Behavioral Health Coordinators in the field.
Conduct psychiatric intakes and psychosocial assessments for
diagnosis and treatment planning.
Coordinate referrals transitions of care and handoffs across the care
continuum.Support audits quality reviews and compliance initiatives.
Travel across a defined region to support 810 facility sites.
Comfort with EHR systems and digital communication tools.
Experience with post-acute long-term care or geriatric populations a plus.
Reliable transportation and willingness to travel regionally.
