Job description
This is a union eligible schedule for this position is: Monday-Friday 8:00am-4:30pmHealthRIGHT 360s Mobile and Street Medicine program delivers comprehensive high-quality medical care and social services to individuals experiencing homelessness in San Francisco. Rooted in the mission to go to the people the program offers services directly to patientson the street in encampments or via a fully equipped mobile medical van. The program operates as an extension of our Integrated Care Center (ICC) expanding the reach of our services beyond the clinic walls. This new and rapidly growing initiative is committed to providing low-barrier trauma-informed and person-centered care in the environments where people live. Our multidisciplinary team prioritizes trust continuity and access while delivering compassionate dignified and high-quality services to some of the citys most underserved Community Outreach Driver is a key member of the mobile medicine team responsible for supporting community outreach harm reduction and transportation services. This role involves working as part of a multidisciplinary team to engage individuals experiencing homelessness and connect them to essential medical and social services aimed at improving health and quality of life. The Community Outreach Driver will operate the mobile medical van provide safe transportation for the outreach team and support team safety in the field. Additionally this role includes direct engagement with clients offering harm reduction education and supplies and assisting with basic case management tasks in collaboration with the case managers. As a trusted presence in the community the Outreach Driver also serves as a street guide helping the team navigate encampments and other field settings effectively and RESPONSIBILITIESTransportation & Vehicle Maintenance:Retrieve the mobile medical van from the Treasure Island vehicle storage facility each morning and deliver it to the designated clinical site; return the van to storage at the end of each the mobile medical van for daily clinical operations ensuring equipment and supplies are ready for transport team members using the mobile medical van or other company vehicles adhering to all applicable federal and state traffic transport clients in company vehicles when needed following all legal and organizational transportation routine vehicle maintenance checks including daily inspections and refueling and replenish fluids such as oil and windshield washer fluid as scheduling and transportation of the mobile medical van and other assigned vehicles to mechanics for routine service or the cleanliness and professional appearance of all assigned vehicle interior and exterior on a regular and maintain up-to-date parking permits for the mobile medical van ensuring compliance with city regulations and appropriate use at both clinical sites and the storage and Engagement: Conduct outreach to identify and connect with individuals experiencing homelessness building trust to encourage their participation in healthcare culturally sensitive health education and information helping clients understand their health conditions and treatment for clients needs within the healthcare system and community resources assisting them in navigating complex care systems and accessing essential coordination between healthcare providers social services and community resources to ensure clients receive comprehensive and timely health promotion activities and programs tailored to the needs of the homeless clients with respect and dignity; interact with them in a caring non-judgmental manner utilizing harm reduction and trauma-informed care some Enhanced Care Management (ECM) case management services when required including referrals care coordination and follow-up support in collaboration with the rest of the team. Offer substance use counseling and support to clients as needed helping them navigate their recovery support to clients with mental health needs assisting them in accessing appropriate services and offering emotional supportSupport harm reduction essential outreach supplies including basic needs items such as water food and hygiene products to individuals experiencing with all health and safety protocols especially against appropriate staff/client and Team Support: Work collaboratively with a multidisciplinary team to deliver holistic care. Act as a street guide for the team leveraging expertise in navigating both the geographical and social landscapes of different neighborhoods. Ensure safe passage and provide transportation vigilance to ensure the safety of the team intervening when necessary to address any potential risks or issues in a supportive and respectful de-escalation techniques to manage and defuse potentially volatile situations ensuring the safety and well-being of both clients and team to medical emergencies including overdoses in collaboration with the rest of the :Maintain mobile medical van and other assigned vehicles use logs and accurate vehicle documentation with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluation care promptly according to charting protocols and clinic policies in the electronic medical record (EMR). Maintain accurate and timely records of patient outreach attempts and visits with geolocation in a shared spreadsheet. Organize outreach items and harm reduction supplies in the office or warehouse keeping track of inventory and identifying needs for and participate in team meetings and case training courses and conferences assigned by the all assigned training in a timely Certification and ExperienceHigh school diploma or equivalent accreditation or CPR & first aid California Class C drivers license access to registered and insured transportation and a clean driving meet HealthRIGHT 360s motor vehicle record (MVR) least 2 years of experience in community health outreach or a related field with experience working with homeless or vulnerable populations is highly and comfortability driving large vehicles (RV-size).Desired:Valid California Class A or B drivers in registration or certification is strongly Specialist Certification or Substance Use Disorder Counselor certification is a plus. Professional driving experience desired.
