Position Summary

The LPN/RN Unit Manager & Wound Care Nurse is responsible for overseeing the daily clinical operations of an assigned nursing unit while providing comprehensive wound prevention, assessment, and treatment services. This position promotes high-quality resident care, ensures compliance with federal and state regulations, and serves as a clinical leader and mentor for nursing staff.

Essential Duties and Responsibilities

Unit Management

  • Supervise and support nursing staff on the assigned unit.

  • Ensure residents receive quality, compassionate, and individualized care.

  • Monitor resident conditions and promptly communicate significant changes to physicians, families, and the interdisciplinary team.

  • Coordinate admissions, transfers, and discharges.

  • Assist with staffing, scheduling, and daily assignment of nursing personnel.

  • Conduct rounds to ensure quality of care and regulatory compliance.

  • Monitor documentation for accuracy, completeness, and timeliness.

  • Participate in care plan meetings and interdisciplinary team conferences.

  • Mentor, educate, and coach nursing staff to promote clinical excellence.

  • Investigate incidents and assist with corrective action plans as needed.

  • Promote infection prevention and safety initiatives throughout the unit.

Wound Care Responsibilities

  • Perform comprehensive skin assessments on admission and routinely thereafter.

  • Assess, stage, document, and monitor all wounds in accordance with facility policies and clinical standards.

  • Develop and implement individualized wound treatment plans.

  • Perform wound treatments, dressing changes, and other procedures as ordered.

  • Collaborate with physicians, nurse practitioners, therapy, dietary, and other disciplines to promote optimal wound healing.

  • Monitor residents at risk for pressure injuries and implement preventative interventions.

  • Maintain accurate wound documentation, measurements, and progress photographs (when applicable).

  • Track wound healing outcomes and quality indicators.

  • Educate residents, families, and staff on wound prevention and treatment.

  • Ensure wound care supplies are appropriately stocked and utilized.

  • Assist during regulatory surveys related to wound care practices.

Qualifications

  • Current, unrestricted LPN or RN license in KY.

  • Current CPR certification.

  • Wound Care Certification (WCC, WCN, CWCN, or willingness to obtain) preferred.

  • Previous long-term care or skilled nursing experience preferred.

  • Prior supervisory or leadership experience preferred.

  • Strong knowledge of wound care principles, infection prevention, and nursing standards.

  • Excellent communication, leadership, organizational, and critical thinking skills.

  • Proficiency with electronic medical records (EMR).

Physical Requirements

  • Ability to stand, walk, bend, lift, and assist residents throughout the workday.

  • Ability to lift up to 50 pounds with assistance when necessary.

  • Ability to respond quickly during resident emergencies.

  • Frequent use of computers, documentation systems, and medical equipment.

Salary/ Wage Range

Compensation for the role will depend on several factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
