Job description

Overview Habitat Health is a Program of All-Inclusive Care for the Elderly (PACE) provider that empowers older adults to experience more good days in their homes and communities. Our mission‑driven care teams help participants live well on their own terms and we are expanding a scalable, affordable PACE model to meet growing needs. Location Remote, NorCal (Sacramento/San Leandro area), up to 75% travel. Required RN License Active Registered Nurse license required in the region/state; a compact license is a plus. Role Scope The Regional Training Lead supports onboarding and training of Habitat team members on‑site at PACE Centers. The role acts as an extension of the Learning & Development hub, overseeing practicum and shadowing activities for all roles at the center, conducting competency reviews, and ensuring team members are cleared to work independently with participants. Core Responsibilities & Expectations Field‑based owner of training fidelity within the assigned region. Supports the on‑site learning experience for Center team members. Manages practicum and shadowing assignments and logistics for new team members. Collaborates with Center leadership to oversee completion of competency sign–offs. Performs scheduled visits to all assigned centers, supporting leaders and Center Champions with ongoing learning needs. Partners with the central L&D hub to design and coordinate on‑site learning opportunities that support role readiness and capability development. Works with SMEs, Center Leaders, and People Team partners to ensure learning programs meet business needs and reinforce Habitat’s culture and capabilities. Required Qualifications Unrestricted RN license within the required region/state; compact license a plus. 3–5+ years of teaching or training experience in the field. Strong facilitation, communication, and coaching skills. Experience delivering learning programs and using structured frameworks. Strong relationship‑building and stakeholder management skills. Alignment with Habitat’s purpose and values; enthusiasm for living those out daily. Ability to thrive in a fast‑paced, evolving environment, navigating ambiguity and contributing to continuous improvement. Growth mindset; seeks feedback, engages in healthy debate, and uses data and curiosity to inform decisions. Acts with integrity and ownership, considering broader organizational impact and delivering results. Nice to Have Experience working in a value‑based care or elder care/PACE environment. Essential Functions & Physical Requirements Work in healthcare, administrative, community, or remote environments as needed. Communicate effectively with participants, caregivers, team members, and other stakeholders in person, by phone, and electronically. Remain stationary and/or move about for extended periods, consistent with job duties. Operate standard office equipment and/or clinical tools (computers, phones, documentation systems). Travel between work sites or community locations as required. Follow safety procedures, infection control protocols, and use PPE when required. Perform tasks that may involve bending, reaching, lifting, or assisting participants. Lift, carry, push, or pull supplies, equipment, or materials typically up to 25–50 pounds, with assistance or mechanical aids. Compensation & Benefits Base salary range: $112,000 – $128,000. The position also includes a target annual bonus of 10% of base salary, with potential additional incentive compensation.
