Job Description
The Social Services Assistant is responsible for planning developing organizing implementing and directing the SNF Health & Wellness in accordance with current existing federal state and local regulations as well as Stellar Senior Living established policies and procedures to ensure medically related social services are provided to all residents in the facility.
Responsibilities
- Plan organize develop the overall operation of the Social Services department to ensure all medically related emotional and social needs of patients are met
- Ensures the quality of social services rendered by the facility at all times
- Participate in the admission discharge MDS coordination etc for social services
- Refers patients and families to appropriate community resources as necessary
Qualifications
- Must be currently registered/ licensed in the State
- Bachelors Degree in Social Work or equivalent
- At least two years of experience in Long Term Care preferred.
- Demonstrated knowledge or rules regulations and guidelines that govern nursing care facilities.
