You will educate patients and families and maintain patient safety while delivering comprehensive skilled services as ordered by the attending physician. MAJOR AREAS OF RESPONSIBILITY Plan of Care Participate in the development of the Plan of Care, conduct assessments of patient needs, identify barriers, observe, record, and report patient reactions or changes to appropriate agency staff. Patient Outcomes Connect patients and families to community resources, assist with patient financial aid applications and long‑range planning. Family Educator/Advocate Counsel patients and families about coping, changes, and grief. Policies Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Rules and Regulations Understand and follow agency policies, procedures, rules, and regulations. Communicate changes in schedule or availability to schedulers or supervisors. Operations Attend in‑service training and mandatory agency meetings. HARD & SOFT SKILLS Compassionate communicator with a positive attitude. Patient and attentive to detail when working with patients, families, physicians, and coworkers. REQUIREMENTS Medical Social Worker with a current license in the state of employment (limited licenses not accepted) Minimum one‑year experience as a MSW in an acute care setting; home care experience preferred Master’s degree in Social Work Valid driver’s license and personal auto insurance (for driver role) Capable of all physical demands required for the position We are an Equal Opportunity Employer.
