Licensed BSW Social Worker

In this role, you will be responsible for delivering high-quality, evidence-based social work services to our residents, focusing on their physical, emotional, and social well-being. Licensed BSW Social Worker is a key member of our interdisciplinary team, collaborating with healthcare professionals to develop and implement personalized care plans.
Responsibilities:
• Assess and address the social, emotional, and mental health needs of our residents, working closely with healthcare professionals to develop and implement comprehensive care plans
• Provide counseling and supportive services to residents, families, and caregivers to promote emotional well-being and social support
• Facilitate and participate in interdisciplinary team meetings to ensure seamless coordination of care
• Develop and maintain positive relationships with residents, families, and staff to promote a culture of respect, empathy, and compassion
• Maintain accurate and timely documentation of resident assessments, care plans, and progress
