Job description
A company is looking for a Palliative Care / Hospice Clinical Specialist to provide program support and coordinate care for members enrolled in Palliative Care / Hospice.
Key Responsibilities
Coordinate care for Palliative Care / Hospice members and engage with families and caregivers
Facilitate advanced care planning discussions and collaborate with healthcare providers
Monitor vendor performance and ensure smooth transitions between care settings
Required Qualifications
Current state RN Licensure required
Associate's Degree in Nursing required
2+ years of experience in Palliative Care and Hospice required
CA RN License required
