Job description
To support a growing practice, the part-time Michigan Licensed Advanced Practice Clinician will provide flexible virtual care coverage for longitudinal care pods, managing inbox triage, conducting patient assessments, and collaborating with Health Advocates to ensure continuity of care. Key responsibilities Conduct comprehensive virtual onboarding, follow-up visits, and manage complex chronic conditions Dedicate hours to inbox management, urgent clinical triage, and results follow-up for covered panels Support preventative-care quality campaigns and coordinate with multidisciplinary teams for complex patient care Required qualifications Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license Active NP/PA licensure in Michigan, Massachusetts, or Colorado, with willingness to obtain additional state licenses Active DEA registration and state controlled-substance license Current BLS certification 3+ years of experience in adult primary care, family medicine, urgent care, or emergency medicine
