Job description
To enhance patient care, the full-time remote Ohio Licensed Ambulatory Care Pharmacist will provide Comprehensive Medication Management (CMM), collaborate with healthcare teams, and develop individualized therapeutic regimens for chronic disease management. Key responsibilities: Provide Comprehensive Medication Management (CMM) in collaboration with clinical care teams Develop medication management action plans to achieve member self-management goals Communicate with internal and external providers to align clinical interventions with medication-related goals Required qualifications: PGY1 pharmacy practice residency and PGY-2 Ambulatory Care Pharmacy Residency, or Minimum of 2 years' experience as a clinical pharmacist in an ambulatory care setting Active Ohio pharmacist license Experience in developing clinical pathways and protocols Ability to conduct medication assessments and initiate or adjust medications per protocol
