To support patient care initiatives, the full-time California Licensed Vocational Nurse Coordinator will manage a panel of 400-450 patients, providing health coaching and conducting monthly outreach in a remote work environment. Key responsibilities Conduct 50-60 daily outbound calls to patients, ensuring continuity of care and providing tailored health coaching Collaborate with clinical teams and caregivers to address patient needs and close gaps in care through various management services Document all patient interactions in real time while maintaining HIPAA compliance and supporting care coordination goals Required qualifications Active California Licensed Vocational Nurse license or multi-state compact license Minimum of 2 years of clinical experience in a healthcare setting Experience with Electronic Medical Records (EMRs) and strong computer navigation skills HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability Must be legally authorized to work in the United States
