RN Case Manager — Stoughton Health (Part-Time) Stoughton Health offers an excellent opportunity for an RN Case Manager to join our Medical‑Surgical team on a part‑time basis. The RN Case Manager will carry out many duties that directly affect customer service, quality care, and hospital reimbursement. This multi‑focal role involves reviewing benefits for planned admissions, concurrently reviewing charts for medical acuity, coordinating admissions to supportive care, participating in discharge planning within the multidisciplinary care team, assisting in assignment of DRG’s, and contacting hospital departments and outside agencies. Position Requirements Required Education: Associate’s Degree in Nursing or experience commensurate to a Bachelor’s degree Preferred Education: Bachelor’s Degree; advanced computer coursework Professional Experience: Five years acute‑care nursing experience; preferred prior related work experience in Patient Review or Case Management Licensure: Currently licensed by the Board of Licensing and Regulation in the State of Wisconsin as a Registered Nurse in good standing Computer Competency: Basic computer skills expected for this position Microsoft Office Suite Skills: Beginning skills – able to perform basic tasks such as opening a document, changing font, saving a document, writing a letter Work Shift Part‑time, .5 FTE Week 1 – 24 hours Week 2 – 16 hours Scheduled Days: Variable between Monday and Friday Shift: 8:00 a.m. – 4:30 p.m. No weekends Benefits 403(b) retirement plan (employer match at hire) Medical insurance (multiple health plan options) Dental and vision insurance Disability and life insurance Flexible spending account Health savings account (HSA) Paid time off (PTO at hire) Employee assistance programs Tuition assistance Free on‑campus parking Work Environment This position is an in‑person role at our Stoughton Hospital location.
